Billing

I, the renter, understand the initial monthly rental fee be it month to month or a prepaid package is paid the day the order is placed (Ohio residents subject to state and local sales tax).

Upon completion of the initial rental term, be it Month to Month or a Prepay & Save package, the additional rental fees, depending on the model (Ohio residents subject to state and local sales tax) will be billed;

If the doppler is returned prior to completion of a full monthly term, based on calander month, then the customer will be billed based on the daily rate or the monthly rate, which ever is less.

Daily Rentals rates are $.84 per day for non-display fetal dopplers and $1.40 per day for display fetal dopplers. The minimal rental contract is one month. FetalSure will provide a fee schedule with every rental.

Fetalsure will email an invoice to the email address provided at the time of rental, if the email is invalid the invoice will be mailed. An online payment link will be provided to allow the renter to pay online. You will have 7 days to pay the invoice. If the invoice is not paid within 7 days your account will be automatically charged.

The initial rental fee will be charged the day the order is placed.

Responsibility and Return of Unit
|

I, the renter, accept full responsibility for the fetal doppler heart monitor while it is in my possession and I agree to be responsible for the replacement at full replacement value for the equipment rented under this agreement but not returned to FetalSure, a division of Bridgeshore, LLC.

I agree to return the equipment rented in clean working condition. Damaged equipment will be charged accordingly.

Please re-pack the unit in the original shipping container. Take the container to the nearest post office, give the package to your postal carrier, or call the toll free number on the box to schedule a pick-up. FetalSure prepays return postage.

Governing Law

Release from Liability
|

I, the renter, hereby acknowledge by renting the Fetal Doppler Heart Monitor from FetalSure, a wholly owned subsidiary of Bridgeshore, LLC, that I hereby release and forever discharge FetalSure, the authorizing physician, certified nurse, midwife and their respective officers, directors, shareholders, employees, agents, representatives, affiliates, successors and assigns (collectively, the Released Parties) of and from any and all liability for any and all loss, harm, damage or claim of any kind arising from or relating to the use of the Fetal Doppler Heart Monitor. I, the renter acknowledge that by this release I am giving up any right now or in the future to sue or otherwise seek monetary damages or other relief against any of the Released Parties for any reason relating to the rental of and use of the Fetal Doppler Heartbeat Monitor.

Acknowledgement of Proper Use
|

The Fetal Doppler does not replace the need to monitor the child's progress with a professional caregiver. If you have any concerns about your own health or the health of your baby always consult with a physician or other healthcare professional. Parents will not use the Fetal Doppler Heartbeat Monitors in any way for diagnostic or other medical purposes. FetalSure Medical Grade Fetal Doppler Heartbeat Monitors are not a substitute for regular prenatal care by medical practitioners.

FetalSure Medical Grade Fetal Doppler Heartbeat Monitors are provided for home use by expecting mothers with the approval of their medical practitioner. Please discuss the use of the FetalSure Fetal Doppler Heartbeat Monitor with your medical practitioner before renting. I the renter verify the user of this Doppler has/will have authorization from a licensed practitioner to use a Fetal Doppler.

Warranty

Upon receiving the Doppler, if you are not completely satisfied contact FetalSure at 1-800-598-3487 or e-mail us at returns@fetalsure.com and return the Doppler within 7 days of receipt for a full refund.

FetalSure warrants that the Doppler will perform to standard specifications during the rental period. If any unit fails during the rental period, FetalSure will replace it free of charge.

Upon inspection of the doppler, if Fetal Sure determines that the Doppler was altered or repaired by anyone other that a authorized technician, or if the unit was broke due to misuse, neglect, or accident, the customer assumes full financial liability for repair or replacement of the Doppler. If the Doppler is lost, stolen, or destroyed the customer will be charged the full replacement value.

I have read and understand this agreement and agree to the terms of the agreement upon renting the Fetal Doppler Heart Monitor from FetalSure, a wholly owned subsidiary of Bridgeshore, LLC.
