The purpose is to ensures that mothers will have professional guidance to use fetal dopplers appropriately to contribute to the well-being of both themselves and the fetus.

Please discuss the use of a fetal doppler at home with your caregiver prior to ordering.
We will ship the doppler upon your order, we do this with the understanding that you have discussed the use of the doppler with your caregiver and received an authorization to use the doppler at home.
Please follow-up with one of the 5 suggestions listed below;
1.Have your caregiver fax one of our authorization forms.
2. Have your caregiver fax a note on their letterhead or fax cover sheet letting us know it is okay for you to rent a doppler for home use. Our Fax number is 1-866-790-5168
3. Have your caregiver's office call our toll-free number to let us know that it is okay for you use a fetal doppler at home, 1-800-598-3487.
4. Have your caregiver fax the authorization form provided with the doppler.
5. Mail a copy of the authorization form provided with the doppler to FetalSure ~ PO BOX 397 ~ Canfield ~ OH ~ 44406
